Hormonal skin changes treatment

Pregnancy Skin Care Routine

People talk about ‘pregnancy glow’, but for many women, that ‘glow’ can feel more like grease, breakouts, dark patches, and sensitivity you’ve never known before.
If that sounds familiar, you’re not alone.

I’m Dr Emma Wedgeworth, Consultant Dermatologist and mum, and I’ve spent years helping women navigate the rollercoaster of pregnancy skin. Those hormonal surges that come with pregnancy play havoc with your complexion. One week you’re radiant, the next you’re battling pigmentation, dryness, or sensitive skin flare-ups that feel like you’re a teenager again.

The good news?

With the right knowledge and a few smart tweaks, you can absolutely move past dull skin days and enjoy calm, healthy, glowing skin throughout your pregnancy.

In this post, I’m going to share my complete, evidence-based, pregnancy-safe skin care routine, the very same approach I use with my own patients.

So whether you’ve just seen that positive test, are well into your third trimester, or simply want to future-proof your skin for the journey ahead, this is your ultimate dermatologist-approved guide to the perfect pregnancy skin care routine.

Why Pregnancy Changes Your Skin (and What’s Really Going On Beneath the Glow)

You’ve probably heard people say you’ll “glow” during pregnancy, but what no one mentions is why your skin suddenly looks and behaves so differently. One minute it’s luminous, the next it’s breaking out, blotchy, or hypersensitive.

Let’s lift the lid on what’s really happening beneath the surface. When you understand the science behind these changes, it becomes so much easier to make calm, informed choices, instead of feeling like your skin has a mind of its own.

1. The Hormone Rollercoaster

Pregnancy floods your body with surges of estrogen, progesterone, and human chorionic gonadotropin (hCG), the biochemical trio that makes everything from your hair to your mood behave differently. These hormones increase sebum (oil) production, boost pigment activity, and even change your skin’s vascular tone.

That means you might notice your skin getting oilier, breaking out in unexpected places, or developing new pigmentation, often around the cheeks and forehead (hello, “pregnancy mask”). At the same time, others find their dry skin becoming even more pronounced, tight, flaky, and more sensitive than ever.

2. A Surge in Blood Flow

Your body’s blood volume rises by up to 50% during pregnancy, incredible, but it can also bring visible side effects. That rosy flush? It’s real. So are the little spider veins that may appear on your legs or face. Extra circulation means warmth, redness, and a more reactive complexion; your skin literally has more happening beneath the surface.

3. Your Immune System Hits “Reset”

To protect the growing baby, your immune system cleverly adjusts itself to be less reactive. The downside? Conditions like eczema, psoriasis, or rosacea can flare up, or, just to keep you guessing, sometimes completely calm down. Every pregnancy is different, and I’ve seen both ends of the spectrum in my clinic.

4. Stretching, Stress and Shifts in Hydration

Your skin stretches more in nine months than it has in years. That mechanical stress, combined with changing hydration levels, can lead to itchiness, micro-tears in the barrier, and the beginnings of stretch marks (striae). The belly, breasts, and thighs are the usual hotspots. On top of that, fluid retention can make your skin feel puffy, tight, or even a bit uncomfortable.

5. Sensitivity on High Alert

Many women tell me they suddenly can’t tolerate their old skincare favourites, skincare products that once felt silky now sting or burn. This isn’t your imagination. As your barrier function changes and hormones fluctuate, your skin can become reactive to fragrances, essential oils, or strong actives.

Because of all these shifts, your regular skincare routine may not only stop working, but it could even make things worse. The real goal of a pregnancy skin care routine is to balance safety, efficacy, and comfort, giving your skin the support it needs.

Safety First: What to Avoid During Pregnancy (and Why)

When it comes to pregnancy skincare, my golden rule is simple: if in doubt, leave it out.

Your skin is more permeable, your body is working overtime, and there’s no reason to take unnecessary risks, even for the sake of glowing skin. The aim here isn’t to scare you, but to empower you with clarity. There’s a lot of misinformation online, and it can be overwhelming to know what’s genuinely safe.

Let’s cut through the noise. Here’s what I advise my patients to steer clear of, and what to use instead.

Retinoids (Retinol, Retinoic Acid, Adapalene, Tazarotene)

These vitamin A derivatives are some of dermatology’s most powerful anti-ageing and acne-fighting ingredients, but they’re also a definite no during pregnancy. Retinoids, especially oral versions like isotretinoin, have been linked to birth defects and are firmly contraindicated.

Safer swap: Try azelaic acid, niacinamide, or a gentle vitamin C serum for brightening and cell renewal. They’re far milder, effective over time, and pregnancy-safe.

Strong Acids and Peels

High-strength glycolic, salicylic, or TCA peels can irritate and penetrate too deeply during pregnancy, when your skin barrier is already more reactive.

Safer swap: Low-strength exfoliants (like lactic acid or 5–10% glycolic acid) or enzyme-based exfoliators offer a glow without the sting. Always patch-test first and use sparingly; less truly is more right now.

Hydroquinone

This popular pigment-fading ingredient is absorbed systemically at relatively high levels, and there just isn’t enough safety data to justify its use during pregnancy.

Safer swap: Azelaic acid, vitamin C, and tranexamic acid (in topical form) are all excellent, research-backed alternatives for tackling pigmentation and “the pregnancy mask.”

High-Dose Salicylic Acid

Small amounts under 2%, such as in a mild cleanser or spot treatment, are generally considered safe. But high-strength peels or leave-on BHAs can be too aggressive.

Safer swap: Keep salicylic acid use minimum and infrequent. For acne-prone skin, azelaic acid again shines as the safer multitasker.

Strong Fragrances and Essential Oils

Pregnancy can turn your skin sensitive; what once smelled divine might now trigger redness, itching, or nausea. Some essential oils haven’t been fully tested for safety in pregnancy either.

Safer swap: Choose fragrance-free or low-fragrance products, and keep things simple. Your skin will thank you.

Invasive Cosmetic Procedures

Now’s not the time for deep lasers, microneedling, intense peels, or high-energy light treatments. These can overstimulate pigment cells and affect healing, and some technologies simply haven’t been safety-tested in pregnant women.

Safer swap: Stick to gentle facials, hydrating masks, or mild in-office treatments approved by your dermatologist and obstetrician.

Certain Prescription Medications

If you’re managing acne, eczema, or another chronic skin condition, be especially cautious with oral medications such as isotretinoin, methotrexate, or some antibiotics. These can pose risks to the baby’s development.

What to do instead: Always check with your dermatologist and obstetrician before starting or continuing any prescription skincare. There are often gentler, pregnancy-safe alternatives that can be tailored just for you.

In Short: The Pregnancy Skincare Mindset

Think of this as a temporary “skin reset.” Your skin is different right now, softer, more sensitive, and more reactive, and it deserves a calmer, more nurturing approach.

Focus on:

  • Gentle cleansing and hydration
  • Sun protection (your best defence against pigmentation)
  • Simple, well-formulated products with minimal fragrance
  • Evidence-based, pregnancy-safe actives (azelaic acid, niacinamide, vitamin C)

When in doubt, pause before you apply, and if you’re ever uncertain, check with your dermatologist. That one quick question could save you months of irritation (and a lot of stress).

Practical Tips & Troubleshooting

Pregnancy skincare doesn’t need to be complicated, just consistent, calm, and kind to your skin. Here are my golden rules to keep things running smoothly:

  • Patch test everything: Try new products on a small area (like your inner arm) for 3–5 days before using them fully.
  • Simplify your routine: Fewer, well-chosen products work better than an overcrowded shelf.
  • One new product at a time: Wait a week between additions so you can spot any reactions early.
  • Mind your pH: Stick with gentle, slightly acidic cleansers; harsh, alkaline ones can strip your barrier.
  • Hydrate inside and out: Drink plenty of water, eat antioxidant-rich foods, and nourish your skin with hydrating formulas.
  • Manage stress: Cortisol spikes can worsen acne or pigmentation. Prioritise rest, gentle movement, and mindfulness.
  • Avoid extreme heat: Hot showers, saunas, and steam rooms can trigger redness and sensitivity.
  • Choose safe cosmetics: Keep makeup light and mineral-based, and skip new or heavily fragranced products.
  • Check with your OB & dermatologist: Before starting anything stronger, make sure both are happy for you to go ahead.

When to Seek Professional Help

Even with the best routine, pregnancy skin can sometimes take an unexpected turn, and that’s when it’s time to get expert eyes on it. Reach out to your dermatologist (and let your obstetrician know) if you notice any of the following:

  • A sudden appearance of new moles, or changes in shape, colour, border, or bleeding.
  • A painful, spreading rash or blistering that worsens quickly.
  • Severe or uncontrollable acne, especially with pus-filled lesions.
  • Intense itching that keeps you awake can sometimes signal an internal condition.
  • A flare-up of an existing skin disease, such as eczema or psoriasis.
  • Pigmentation changes that are asymmetric, very dark, or rapidly evolving.

In these cases, a proper clinical assessment is essential. Safe, prescription-based treatments are often available, and getting help early can make all the difference.
